DOCUMENTS TO FILE A CLAIM:

* DIRECT PAYMENT TO PROVIDER (SURGERY AND/OR MEDICAL PROCEDURE). Surgery and/or Medical procedure programming: Copy of Insurance Policy. Medical Report that includes diagnosis, place and date of surgery and a detail budget of the participating doctors expenses. Studies that confirm diagnosis. (if exists). Illines or accident notification form fill out and sign by the insured (does not apply for BUPA or BEST DOCTORS). We will receive Authorization letter for that surgery or procedure that will be deliver to the Hospital or Doctor of the insured for direct payment. Medication Programming: Copy of Insurance Policy. Medical Report that includes the dose and period the medication will be taken by the insured. Studies that confirm diagnosis. (if exists). Illines or accident notification form fill out and sign by the insured (does not apply for BUPA or BEST DOCTORS). Budget with medicine cost. They will send an Authorization letter to the assigned provider so they can send the medication to the insured address. * REIMBURSEMENT Copy of Insurance Policy. Medical Report, fill out and sign by your doctor. Studies that confirm diagnosis. (if exists). Illines or accident notification form fill out and sign by the insured (does not apply for BUPA or BEST DOCTORS). Refund form signed by the insured. Wire transfer form fill out & signed by the insured. Copy of Bank Statement that includes account and CLABE number for electronic wire transfer.
